                       FCC Forms Available

The following FCC forms are available for public use:   FCC Form
431, TRS Fund Worksheet;  FCC Form 492, Rate of Return Report; and
FCC Form 492A, Price Cap Regulation Rate of Return Monitoring
Report. Note that the FCC Forms 492 and 492A are to be filed in
triplicate.  File two copies of the FCC Forms 492 and 492A with the
Office of the Secreetary and one copy of the forms with the
Industry Analysis Division of the Common Carrier Bureau.  

FCC Forms 431, 492 and 492A may be obtained as follows:

1.  FCC Forms Distribution Warehouse.  The forms may be obtained
either by writing to the Commission's Forms Distribution Center,
9300 E. Hampton Drive, Capital Heights, Maryland 20431, or by
calling telephone number 1-800-418-3676 and leaving a request on
the answering machine provided for this purpose.

2.  FCC Forms Web Page. Copies of the form may be downloaded from
the Commission's Web Page, www.fcc.gov/formpage.html.
