                        RE-LOCATION OF FCC FORMS ROOM
                                  
                                  
                  The FCC Forms Room is now located at 445 12th St., S.W., Room TW-B200, Washington,
         DC 20554.  This room is located in the Portals lobby, just past the security turnstiles. 

                  The Forms Room formerly located in Room L-17 at 1919 M St., N.W., Washington, DC is
         closed.  

                  Orders for FCC Forms not available in the Forms Room can be obtained by submitting a
         Request for Blank Forms/Bulletins (FCC Form 207) and mailing it directly to the address on
         the postcard or placing it in the tray in the Forms Room to be picked-up by the contractor for
         processing.

                  The FCC's toll-free number for ordering forms is 1-800-418-3676 (outside the Washington,
