The current revision is dated March 1996, and states at the bottom of the form that all previous editions of the form are obsolete. Effective September 1, 1996, the Experimental Licensing Branch of the Office of Engineering and Technology will not accept applications submitted on any Form 442 older than the March 1996 version. After that time, any application submitted on an outdated form will be returned to the applicant with a current form and instructions to resubmit the application on the current form. The Experimental Licensing Branch (ELB) advises all regular Form 442 filers to obtain copies of the March 1996 version of the form as soon as possible. Copies of the current form may be obtained in three ways: 1. Through the Commission's forms hot-line. To request form 442, dial (202- 418-FORM and follow the instructions. 2. Through the fax-on-demand service. To request Form 442 through fax-on- demand, applicants should dial (202) 418-0177 from a fax machine, then at the main menu, key in 000442. 3. By contacting Nancy Hey in ELB's Document Inspection Location. Forms may be picked up at the Document Inspection Location in Room 264A, 2000 M Street between the hours of 9:30 am - 12:00 noon or 2:00 - 4:00 pm.
