               Cable Services Bureau Corrects FCC Form 1240

On December 15, 1996, the Office of Management and Budget approved the Commission's new
FCC Form 1240 (Annual Updating of Maximum Permitted Rates for Regulated Cable Services).  
See Public Notice, OMB Approves FCC Form 1240 (Annual Updating of Maximum Permitted
Rates for Regulated Cable Services), DA 96-7 (January 11, 1996).  An error (described in detail
below) has been found in the Instructions to the initially released paper copy of the form.  This
version of the Form 1240 can be identified by the date "December 1995" found in the lower right-
hand corner of each page.

The error is found only in the Instructions.  It involves three formulas on pages 19 and 20 of the
Instructions:

On page 19, the instructions for Line H4, option 1 contain the following formula:

     [H3*0.1125*(E2/12)]+[(H3+(H3*0.1125*(E2/12)))*((0.1125*E3/12)))].

This formula should read:

     [(H3/2)*0.1125*(E2/12)]+[(H3+((H3/2)*0.1125*(E2/12)))*((0.1125*E3/12)))].


On page 19, the instructions for Line H4, option 2 contain the following formula:

     [H3*0.1125]+[(H3+(H3*0.1125))*(0.1125*E4/12))].

This formula should read:

     [(H3/2)*0.1125]+[(H3+((H3/2)*0.1125))*(0.1125*E4/12))].


On page 20, the instructions for Line H8 contain the following formula:

     [H7*0.1125*(E4/12)].

This formula should read:

     [(H7/2)*0.1125*(E4/12)].

A new release of the paper version of the form has been prepared.  It will be identified by the date
"January 1996" found in the lower right-hand corner of each page.  A computerized version of
Form 1240, which will be available shortly, will not contain this error.
